New Card Agreement
Students Currently Enrolled at SIUE can use the University ID/Cougar Card Agreement Form. Please complete this form and click the submit button. You can upload your photo online. You must come to campus to pick up your card. A government-issued photo ID is required for pick up. We are located in The Service Center in Rendleman Hall, Room 1309. The office hours are Monday thru Friday from 8:00 am to 4:30 pm.
Students Enrolled in Fully Online Degree Programs can use the Non-Photo University ID/Cougar Card Agreement Form. Please complete the form. After you submit the form, please allow 5 to 10 days to process your request. A processing fee of $2 will be applied to your SIUE account.
